Leverage the information below on recommended starting dosages for these patient age ranges:

Adult and pediatric patients ≥4 years

Adult and pediatric patients ≥4 years

Recommended starting dosage:

Per meal
500 lipase units/kg

Per snack
250 lipase units/kg
or half dose of established meal dosage

If symptoms of malabsorption persist, increase dosage

  • Titrate dosage to either 2,500 lipase units/kg per meal, 10,000 lipase units/kg/day, or 4,000 lipase units/gram of fat ingested per day

After starting your patient on CREON, follow up as early as 5-7 days* to assess the need for a dose adjustment1

Dose adjustments should be individualized based on1:

Clinical signs and symptoms

Dosing and administration considerations

  • Fibrosing colonopathy has been reported following treatment with pancreatic enzyme products. Do not exceed the recommended dosage of 2,500 lipase units/kg/meal (or 10,000 lipase units/kg/day) or 4,000 lipase units/g fat ingested/day in adult and pediatric patients greater than 12 months of age without further investigation.
  • Start at the lowest recommended dosage and individualize the dosage based on clinical symptoms, the degree of steatorrhea present, and the fat content of the diet.
  • CREON should not be crushed, chewed, or retained in the mouth. CREON should always be taken with food.
  • If a dose is missed, the next dose should be taken with the next meal or snack. With each snack, administer approximately half the prescribed CREON dose for a meal.
  • CREON should be kept in a dry place and in the original container.

Infant patients ≤12 months

Infant patients ≤12 months

Recommended starting dosage:

Per breastfeeding
3,000 lipase units/120 mL of formula or per breastfeeding

Infant administration

  • Caregivers should administer CREON immediately prior to every feeding.
  • The CREON capsule or its contents should not be crushed or chewed.
  • Do not mix CREON capsule contents directly into formula or breast milk.
  • Contents of the CREON 3,000 lipase unit capsule may be put directly in the infant’s mouth right before feeding with formula or breast milk.
  • The contents of the CREON capsule can be mixed into a small amount of room-temperature, acidic, soft food, like applesauce, bananas, plain Greek yogurt, or select jarred baby food. The entire food mixture should be consumed immediately. Do not store CREON that has been mixed with food.
  • Give the child enough formula or breast milk to completely swallow the CREON capsule contents or CREON food mixture.
  • Care should be taken to ensure that the entire dose is swallowed and not left in the mouth, as it can cause irritation of the oral mucosa.
  • CREON needs to be stored and dispensed in the original container to protect from moisture. Keep the desiccant in the bottle if present.

Help caregivers remember how to appropriately give enzymes to infants.

CREON should be taken with every meal and snack, with sufficient fluid. Capsules should not be crushed or chewed.1
